It's something I designed myself using Pear Stairs who supply stairs including open tread, as here in Hemlock timber. Then for the metal guarding a local firm called B&M Engineers here in Oxford made it from my drawings in powder-coated steel and then fitted it.

Down stairs toilet -where should it go ?

No doubt the under-stair option will be easier/cheaper as i bet the soil pipe runs straight past it and as you say there is water available directly above. However, under-stairs toilets are usually very poky and awful for men standing up and you lose very useful storage space for coats and other paraphernalia. If you can, i would build a proper bathroom in your garage but you will need to insulate walls, create access and possibly run new soil pipes depending on location. Equally, access through other rooms to get to a loo is not ideal. Avoid macerators at all costs, they are noisy, ugly and breakdown constantly!

I would always rather invest in stunning furniture than spend money on more permanent changes. In my experience you can make any space look great with the right furniture and decor. Rather than spend money on something structural I would work with the space you have with furniture that works better than the heavy farmhouse style. The nicest furniture in the room is the sideboard so I would build on that. I'd look at something like Gubi Beetle dining chairs to bring in an organic streak. Not cheap but cheaper than building in storage (the next best option) or enlarging the window which won't make much difference - and you can take them with you if you move! You could look at Made.com for a cheaper but complementary table. Then a decent lampshade - perhaps Tom Dixon Etch - a side lamp on the sideboard and lots of plants and pots to empathise the organic, cosy nature of the room. Finish with some pictures in the wall and you have a lovely, cosy, earthy room that feels like being down Alice's rabbit hole rather than under the stairs. Good luck.

Advice on where to move the staircase and other renovations

Move the stairs so they run alongside the utility wall - replacing the bathroom space. Then you get a hall from the front entry giving direct access to the kitchen. Hall storage can fit under the stairs near the kitchen. Upstairs, I would utilise the old stairwell and landing as ensuite and walk through wardrobe for the master bedroom., if you can sort out drainage ( you may be able to run it down to the downstairs hall and box it out) Expand the old w/c into a bathroom - you may be able to extend the bathroom over the stairwell a bit, if the downstairs stud is high enough. I've attached a picture, but I can't get it to show big unless you click on it....
