Should I fill in gap between house foundation and patio?

Hi
Should I fill in the gap shown in the pics, I can see some black plastic in the cement. Not sure how concerned I should be or if I should fill it.
We do have a slight negative slope towards the house, which I'm hoping to sort by pouring a new slab over the existing to raise it up.
