Infested apple tree

There is a well established apply tree in my garden (not sure of the variety but nice sweet apples).
On he trunk there is an area of exposed bark and evidence of larva that have been growing in it (see photo).
What do I do about it? It is on the trunk so I fear cutting it out will bring the whole tree down.
