Do we need an architect or designer (or both)?

We're looking to swap positions of our bathroom and kitchen in a ground floor 2-bed Victorian flat. Space is quite tight so some clever ideas will be needed to make the most of it. Given it's an old building and we want to extend slightly to the side, should we go with an architect or designer? Are there designers in London who specialise in kitchens AND bathrooms?
