Your designer has tried to achieve a regular arrangement of the lights in relation to the room. There's often a conflict between what might look most balanced in the space and the most practical positions for lights. You should discuss the options with him/her. We usually position spotlights / downlights just in front of the edge of the countertop, so some of the light goes into the base units. We avoid a light directly over the sink, since you're usually casting a shadow over the sink. Here, the lights over the countertop appear to be very near the wall, perhaps to enliven the wall surface with scallops of light.

i cant find a photo (new phone) but i designed a very similar ensuite for my last house. It worked well. i had a walk in, open shower though. With a flush mount track in the ceiling for the shower curtain. But glass would have worked well too. Only suggestions I have is a cavity sliding door would work well there. Also I'd move the doorway up back a little so you have a nib wall where the sink is. However you probably can't fit both of those changes in. If i had to choose i think I'd go for the sliding door, presuming your getting a rounded sink as shown in plan. I'll keep looking for a photo for you.
