Found some damp - what to do next?

Hi everyone - we moved some furniture and discovered a damp wall + mould. What are probable causes and what should we do next?
It's a north-facing wall, 5th floor, with a balcony outside as well. I'm wondering whether it could have come through the wall? There has also been some condensation on the window and balcony door, could this be the cause?
Any advice much appreciated, not sure how to fix this.
