architect or an interior architect?

We’ve bought a house that needs to be gutted and extended. The flow doesn’t work, some walls need to come down but probably some put up. But I confused with who to hire to help. Do I get an architect and then once planning is approved appoint an interior architect/designer or just go straight to an interior architect? Even with an architect I need a structural engineer but would go with hiring an architect or by passing it. Are the firms that do both? We based in Hertfordshire and I’m having trouble finding an all in one company. Any help or advice would be greatly appreciated.
